The thirteen month requirement allows parents with insurance that only covers one physical per twelve months to schedule a physical that overlaps with our requirements. Flag Football and Flag Cheering only require a physical to be turned in the first year that the child participates. Mites, Pee Wee and Jr High Football/Cheering require a physical be submitted each year of participation.</p> <p class="style3"> Doctor&#39;s offices are VERY busy in July/August. Be sure to schedule an appointment far in advance to ensure that your child is able to meet the requirements to participate!</p> <p class="style3"> Please include any special medical information that coaches should be aware of (inhalers, epi-pens, insulin, recent injuries, etc). </p> <p class="style3"> <span class="style5"><strong>BIRTH CERTIFICATES</strong></span>: A copy of a notarized birth certificate <span class="style6"><em><strong>(Pee Wee football and Jr High football only!)</strong></em></span> is due before the athlete may participate in any game or scrimmage. Please black out any social security numbers shown on the copy before turning this in.</p> <p class="style3"> <span class="style5"><strong>INSURANCE</strong></span>: For families without health insurance, the state of Maine offers a program called Maine Care (formerly called Cub Care) which may provide you with an option to obtain medical coverage at little or no cost. Application processing usually takes 6-12 weeks, so please plan accordingly.
