Player Paperwork

To be allowed to participate, each player must have completed each of the following documents and have them submitted to their head coach.

To print the required documents, select a document type to open (PDF, MSWORD, HTML) and click each of the links in that column to open the document for printing.  PDF or MS Word format provide the best printing results.  The Adobe PDF Reader can be downloaded here and can be used to print the PDF version of these documents.

Note:  A copy of a signed doctor's physical is acceptable in place of having the information filled in on the Physical Form.  The Contact Information at the top of the document must be filled out though by a parent/guardian.  Also, please include any special medical information that coaches should be aware of (inhalers, epi-pens, insulin, recent injuries, etc).

For Flag Football, a physical is required to be turned in during the player's first year of play.  Future years of play in Flag Football do not require a new physical, although it is recommended that children involved in a physical sport like Flag Football be examined by a physician each year.
